Community Health Survey
Because we believe you care about our community’s health and well being, we are helping to promote this important community survey opportunity. The coalition, Voices for Health Justice MKE, is eager to know how you view the health of our community in Milwaukee. Your opinions are very important and will help to make sure the communities’ voice is heard in advocacy efforts that encourage Acsension and Saint Joseph’s Hospital to invest further in the community’s health.
For completing this survey, you will be entered into a drawing for one of (5) $100 Amazon gift cards. All of your individual responses will be kept confidential.
